a year-round basis. This is a FREE service for the organizations
but paid for by our website sponsors. Therefore, we ask that you

2) COPY of Birth Certificate of the child
3) Current wallet size photo of the child
$100 per child (does not include physicals)
*Families with more than 2 children pay for the first 2 ($200 max)
**$200 per child who did not turn in equipment last season
***$75 PER CHEERLEADER

Physicals will be provided FREE of charge by NYF. Information for the provided physical will
be given out during registration and at this website on a later date. All players and
cheerleaders must have his or her physicals by August 6th. No one will be allowed to
condition or practice without a completed physical form. If you do not attend the provided
physical, you will need to have a physical completed by your doctor. The physical forms will be
available at sign-ups or print out form from below. If you wish to use a physical that was
provided by the Middle School (and is valid until 2009) you are responsible for obtaining a copy
from the High School Athletic Department (330-825-4167).

This camp is for any girl interested in cheerleading and learning fun cheers,
chants, dances and jumps. The camp is being held by the high school
cheerleaders. The fee includes all materials taught, a camp t-shirt, a snack
each day and a chance to win a trophy. Join the fun!
